Step 1: Complete the USA Hockey Registration

1. Click on the REGISTER ONLINE BUTTON

2. Click on ICE PLAYER & COACH

3. Check box you are 18 or older

4. Select 2010-11 Season

5. Select A Child Family Member

6. Fill in the form for Participant and Player

7. Check box to Accept Waiver

8. Print the Registration Form and bring it to registration with your other registration papers.

9. If Possible - Email the registration to btksm@aol.com

Step 2: Print and complete the following registration forms and bring them to registration:

1. 2010-11 Registration PDF (SIH Form)
2. SIH Hockey Code of Conduct PDF  (USA Hockey)
3. Consent to Treat PDF  (USA Hockey)
   • Must complete Page 2 of the Consent to Treat form
4. Wavier of Liability PDF  (USA Hockey)

Step 3: Additional items needed at registration:

1. Check for $250 - 1st Installment (non-refundable)

2. Copy of Birth certificate
3. Player Picture ID (school ID)
