Registration

Step 1: Complete the USA Hockey Registration

1. Click here to enter USA Hockey's registration site.
2. Click on ICE PLAYER & COACH
3. Check box if you are 18 or older
4. Select 2011-12 Season
5. Select A Child Family Member
6. Fill in the form for Participant and Player
7. Check box to Accept Waiver
8. Print the Registration Form and bring it to registration with your other registration papers.
9. If Possible - Email the registration to btksm@aol.com

Step 2: Print and complete the following registration forms and bring them to registration:

1. 2011-2012 Registration (SIH Form)
2. 2011-2012 SIH Hockey Code of Conduct (USA Hockey)
3. Consent to Treat (USA Hockey)
• Must complete Page 2 of the Consent to Treat form

Step 3: Additional items needed at registration:

1. Check for $250 - 1st Installment (non-refundable)
2. Copy of Birth certificate
3. Player Picture ID (school ID)
